HOW TO COMPLETE REGISTRATION AND TRANSFER FORMS

1. When registering an animal whose sire and dam are both AMZA registered,

fill out Sections 1 and 2.

2. If you are transferring it to a new owner at the same time,

fill out Sections 1 & 2 and 3.

3. If you are transferring ownership of an animal that is already AMZA registered, fill in the animal’s name is Section 1, then fill out Section 2 and 3.

SEND THIS FORM TO AMZA ALONG WITH THE ORIGINAL AMZA REGISTRATION CERTIFICATE.

Transfers for animals that are already registered cannot be done without the original

certificate.

4. If the animal you are registering, is already registered with another registry, fill out

the animal’s name, in Section 1.

Fill out Section 2 and attach a copy of the other registry’s certificate.

5. If the sire and/or dam of the animal you are registering is not registered with AMZA, fill out Section 1 and 2 and attach a copies, front and back of the other registry’s

certificate.

OTHER INFORMATION

1. Who is the breeder?

The breeder is the person who owns the cow at the time of conception.

When registering a cow, you always use the herd name of the breeder.

2. When do I need a breeding memo?

If the sire of the animal you are registering is not owned by the breeder, you must have the owner of the sire fill out and sign a Breeding memo.

You also need a Breeding Memo when purchasing a bred cow in order to register the calf when born.

3. How to register the calves when you purchase a bred cow.

When you purchase a bred cow, the owner should provide you with a signed Transfer Form (this is Section 3 of the Registration Application) for the dam and a Breeding Memo for the sire.

You can transfer the cow at any time.

The Breeding Memo should be submitted with the Registration Application when the calves are born, they will be registered with the herd name of the breeder.

The purchaser of the bred cow will be shown as the owner and the term “Prenatal” will be used as the date of ownership.
